Who is this program for?

This program is for people in Hobart who have spent time in hospital to overcome psychosocial challenges and improve quality of life.

Sometimes—but not always— people become homeless before they are admitted to hospital. If you do not have a home to go to after your time in hospital, it might be helpful to spend time in accommodation that is available through the Choices program.

About the program

Delivered in partnership with the Department of Health, Choices is an evidence-based person-centred program.

Some of the things you might like to work on could include:

  • finding stable long-term housing
  • improving and maintaining your wellbeing
  • connecting with the community.

The Choices program is designed for people who are unable to be discharged from in-patient settings due to homelessness or require intensive support to prevent re-admission.


Where is this program delivered? How can I access the Choices program?

The Choices program operates in Hobart. There are two separate cottages: the Rose Cottage for women and Karingal for men.

Rose Cottage has five bedrooms and living areas.

Karingal has five bedrooms and shared living areas.

Both sites support people who are currently homeless and unable to be discharged from in-patient settings or require intensive support to prevent re-admission. The accommodation provides a homely environment for you to focus on personal recovery.

What will happen when I access this service? What will my experience look like?

Step 1. Meet and greet

One of our practitioners will come and meet you to discuss whether Choices is right for you.

 

Step 2. Entering the program

You’ll receive a welcome pack of information and start safety planning. You’ll also learn about the house rules and become familiar with your room and communal spaces.

 

Step 3. Goal setting

We support you to understand, plan and set your recovery goals as achievable steps.

 

Step 4. Review your plan

After 6 weeks, we’ll review your goal plan together. Here, you can tell us if you have more goals you’d like to work towards, or if you’d like to explore other supports.

 

Step 5. Transition to the community

Together, we’ll review your goal plan again. If needed, we can help you get in touch with other supports. At this stage, we also ask that you give us feedback, so we can continue to support others and provide the best service possible.


Important information

Please note the following information about the Choices program:

  • there are no overnight visitors
  • we ask that you follow all house rules and attend meetings
  • we are NOT a Crisis Support Program and do not provide clinical support. For emergencies, please dial 000
  • Mental Health Practitioners are not available 24/7 but will visit you frequently.
