Support for severe and persistent mental ill health: Foundations

Who is this program for?
Foundations is a state-wide program for people in Tasmania with severe and persistent mental ill health. Specifically, the program is designed for people who:
- are 18-64 years old
- are case managed through the Department of Health Mental Health Services (also sometimes called MHS)
- do not require 24-hour support.
Foundations works one-on-one with people to help you recover, make progress and build important life skills.
About the Foundations program
Foundations is a nationally accredited, intensive psychosocial recovery-based outreach program. It offers tailored packages of care and is delivered in partnership with the Department of Health.
The Foundations program is person-centred, and strengths focused. Care is tailored to each person’s specific needs, goals and situation.
Foundations encourages people to engage their family, carers and community in the program if they choose to do so.
The program supports you to have more control in your life. This is done by building skills, identifying resources that will help meet your needs, and supporting you towards your recovery goals.
Wrap-around support to achieve your personal goals
Foundations is designed to help you achieve your unique personal goals. This can include things like:
- finding and keeping accommodation
- learning independent living skills
- improving social connection
- improving your overall social and emotional well-being
- connecting to the community
- increasing independence
- remaining well.
Where is this service delivered? How can I access Foundations?
Foundations is delivered Tasmania-wide.
Referrals to this service are made through MHS Case Management. People can be re-referred to the program if required.
What will happen when I access this service? What will my experience look like?
While everyone’s experience of Foundations will be unique to your personal situation, there are some common steps that every participant goes through.
Throughout this experience, your Mental Health Practitioner is your main point of contact. They will work with you and your Case Manager.
Step 1. We’ll arrange a meet and greet
You are welcome to bring family or other key support people. In this meeting, we’ll discuss the Welcome Pack, chat about the program and what it looks like for you, and go through program information in detail.
Step 2. Discovery and planning, together
This involves identifying the recovery goals you’d like to achieve. It also includes discussions around what keeps you well and supports your wellbeing.
Step 3. Working towards your goals
In this step, we identify strategies to support your goals.
Step 4. We review your goals
It’s important to reflect on your goals, the program so far, and any resources, strategies, or skills you have learned. This ensures we can provide the best support possible. It also provides an opportunity to discuss any new goals.
Step 5. If you’ve achieved your goals, we plan for independence
We’ll collaboratively answer the question: ‘Where to from here?’ This might include developing new goals or connecting you with new programs. We also invite you to give us feedback, so we can improve moving forward too.
The Foundations program generally runs for a period of 3-18 months depending on your personal goals and situation. We can also adjust hours and support resources as your needs or circumstances change throughout the program.
